Social prescribers are available when you need extra support and advice. They act as a gateway service and can direct you to non-clinical services, community groups, and support in the local area who can give you practical and emotional support. 

They can help with a range of issues, including:

  • Loneliness
  • Isolation 
  • Grief
  • Work, finances, and money issues
  • Carer support
  • Depression
  • Mobility

Our social prescribers will support you to identify what matters to you and will create a plan to move forward. They will work on underlying issues that affect your health and wellbeing, and will connect you to other appropriate services such as local community groups and activities such as befriending, gardening, support with healthier eating, exercise, and debt guidance. Social prescribers will help you improve your confidence and skills to self-manage and support you in setting personal goals.
